License Renewal Requirements

  1. Duly accomplished form
  2. Two (2) pcs. passport size pictures: close up, colored, plain background
  3. Photo/xerox copy of recent professional ID card
  4. In case of LOST professional ID card which is still current, the applicant shall submit notarized Affidavit of Loss
  5. In case of DESTROYED professional ID card, the applicant shall surrender the destroyed card

Procedure (We help PICPA members in renewing their professional licenses, just pay the Handling Fee of Php100.00 and no need to follow the procedure below )

  1. Present duly accomplished form together with the requirements at:
  2. UNIT A - Window 16       UNIT B - Window 19       UNIT C- Window 30

  3. Pay prescribed fees at the Cashier
  4. Get your claim slip adjacent window
  5. Claim your professional license as scheduled. Please refer to your claim slip for further instructions.

Petition for Change of Registered Name due to Marriage Requirements

  1. Duly accomplished Petition Form that is notarized or under oath.
  2. Certificate of Marriage/Marriage Contract duly certified by the National Statistics Office (in NSO security paper).
  3. Marriage Contract issued by Shariah Court and authenticated by the National Statistics Office for Muslim Rites; and Certificate of Live Birth issued by the NSO showing the conversion of Christian Name to Muslim Name, if necessary.

     

    Marriage Certificate duly authenticaed by the Philippine Consulate Office abroad for Marriages outside the Philippines.

  4. One (1) copy of 1 1/2 x 1 1/2 picture
  5. Statutory fee of P225.00
  6. P15.00 worth of documentary stamps.

Procedure (We help PICPA members in renewing their professional licenses, just pay the Handling Fee of Php100.00 and no need to follow the procedure below )

  1. Fill-up petition form (available at the office of the Assistant Secretary, 3rd Floor main building or at the Customer Relations Counter, PRC Ground and PICPA MMR Secretariat Office.
  2. Go to the Legal Division (2nd Floor, main Building) for notarization of your petition.
  3. Proceed to the Registration Division, ground Floor, main building for verification of your registrered name and for assessment of fees.
  4. Bring the documents to the Records Section, ground floor, annex building for verification of Examination Records.
  5. Go to the cashier for payment of fees.
  6. Submit duly accomplished petition form to the Office of the Assistant Secretary. Petition form should be accompanied with a certified true copy of your Certificate of Marriage/Marriage Contract. (Note: registered name/date of birth should tally with your name/date of birth printed in the Marriage Contract. Should there be a discrepancy between the two, submit a certified true copy of your Certificate of Live Birth/Birth Certificate.
